Do You Know about HEARTS Respite?

UCPWA has been awarded a grant from the Children's Trust Fund to provide reimbursement for respite (temporary relief) to caregivers of children with disabilities under the age of 19 in Congressional District 7. HEARTS (Help! Emergency And Respite Treatment Services) is designed to allow families to choose their own caregivers, offering flexibility of when, where and how much respite is provided. Families will need to complete Voucher Respite Enrollment and Demographic Data forms and return them to UCPWA. After approval, funding may be available on a per-quarter basis, on a first-come-first-served basis until funds are depleted. Disability must be documented by either a doctor's statement or copy of IEP indicating the child's disability. The application process is very simple.
